New clients,  Please click the form link above to download the "New Client Intake Form". You can either print it out then fill it out by hand, or save it to your computer and open it in Adobe Reader directly to utilize the app's 'Fill & Sign' feature to complete the form electronically. You can then save and print the completed form  and email it to me or you can bring it with you for your
first session and consultation.
